Module 7 · Incident Reporting · 3-Tier Framework

Module 7 of 8 · Safe classrooms

Protect students. Get help. Report facts.

Your goal: recognize when to act immediately and maintain supervision while help arrives.

Immediate response

  1. Stop the unsafe activity. Keep the group supervised and account for students.
  2. Get designated site help immediately. For an emergency, follow the site emergency plan and contact emergency services when needed. Do not delay urgent help while trying to reach operations.
  3. Notify Kodely through the urgent route. Confirm that the message reached a responsible person.
  4. Document facts promptly. Record time, place, what you observed, exact relevant words, actions taken, and who was notified.

Situations needing immediate site attention

  • A student is missing, unaccounted for, or leaves the supervised area.
  • An injury, head impact, illness, or possible allergic reaction needs assessment.
  • A physical fight, immediate threat, or unsafe tool/material use occurs.
  • A student discloses harm or another serious safety concern.
  • An unauthorized person attempts pickup or access to students.

Follow your required safeguarding training and applicable reporting duties. Internal reporting does not replace any required external report. Do not investigate a disclosure, ask leading questions, or promise secrecy. Listen calmly, preserve the student’s words, and involve the designated safeguarding staff promptly.

Everyday supervision

  • Use attendance and head counts at transitions; agree who supervises each area.
  • Never leave students alone to find materials or chase a missing student. Call for help while the remaining group stays supervised.
  • Stay visible to another authorized adult; do not meet alone with a child behind a closed door.
  • Confirm authorized pickup and complete the site’s sign-in/out process.

Pause and practice

A co-leader thinks a child went to the bathroom, but no one can confirm where the child is. What happens next?

Check your response

Treat the child as unaccounted for: alert designated site staff immediately and activate the site’s response procedure. Maintain supervision of the remaining group, coordinate the search with authorized staff, notify Kodely, and document the facts. Do not wait until the session ends.

Before marking this lesson complete: explain the response in your own words. If anything is unclear, ask onboarding@kodely.io before continuing. The practice reveal itself is not a scored answer or a recorded signature.
